About Our Practice
FHCL Psychiatry is a psychiatric practice operated by Ariel Greene, a licensed Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) in the state of Connecticut. We provide comprehensive mental health services including psychiatric evaluations, medication management, and ongoing psychiatric care.

We provide services to children (6+), adolescents, adults, and elders (65+).
Appointment Policies
Scheduling
- Appointments must be scheduled in advance
- Initial consultations are required before ongoing treatment
- Telepsychiatry appointments are available for established patients
- Emergency services are not provided through this practice
Cancellation Policy
- 24-hour notice required for appointment cancellations
- Late cancellations (less than 24 hours) may incur a fee
- No-show appointments may result in discharge from practice
- Multiple missed appointments may require a new evaluation
Payment and Insurance
Accepted Insurance
We accept most major insurance plans including Aetna, Anthem BCBS, Carelon Behavioral Health, Cigna (Evernorth), Medicare, Medicaid (Husky), Optum, United Healthcare, Oxford, and Connecticare. Please verify your coverage before your appointment.
Payment Responsibilities
- Copays and deductibles are due at the time of service
- Self-pay patients must pay in full at each visit
- Payment methods accepted: cash, check, credit card
- Outstanding balances may result in suspension of services
Medical Records and Privacy
Your medical records are confidential and protected under HIPAA regulations. We will:
- Maintain accurate and complete medical records
- Protect your privacy and confidentiality
- Provide copies of records upon request

Medication Management
Prescription Policies
- Controlled substances require regular monitoring
- Medication changes require follow-up appointments
- Early refills are generally not provided
- Lost or stolen medications must be reported immediately

Emergency Situations
This practice does not provide emergency services. In case of a mental health emergency:

Discharge Policy
We may discharge patients from our practice for the following reasons:
- Non-compliance with treatment recommendations
- Multiple missed appointments
- Inappropriate behavior or threats
- Non-payment of fees
- Need for specialized care beyond our scope
Limitations of Service
- We do not provide 24/7 emergency coverage
- We do not provide inpatient psychiatric services
- We do not provide court-ordered evaluations
- We do not provide disability evaluations

- Respect the privacy of other patients
- Do not share false or misleading information
Prohibited Activities
